**Position Overview:**
The Home Care Specialist will provide care coordination, navigation, and care flow management services to clients involved in the Hospital@Home and OHT programs. This role acts as the liaison between home care clients, hospital partners, and community care teams, ensuring a seamless transition to the client’s home environment.

**Key Areas of Accountability:**
- Receive, review, and accept referrals for in-home transition services.
- Serve as the primary contact for the hospital navigator/coordinator.
- Coordinate the delivery of home care services and medical supplies as needed.
- Monitor and update client tracking dashboards.
- Liaise with hospital leads and community support services.
- Participate in hospital discharge planning meetings and client care conferences.
- Facilitate decision-making regarding appropriate services with clients, families, and healthcare teams.
- Ensure seamless navigation of community services and Primary Care Physician connections.
- Occasionally provide direct client care and train staff on delegated tasks.
- Monitor and report outcomes of patient/family care conferences to hospital partners and program teams.
- Address client complaints or issues and communicate them to program leads.

**Required Qualifications:**
- Diploma or Undergraduate degree in Nursing (RN Licensure required).
- 5+ years of experience in home and community health or a related field.
- Member in good standing with the College of Nurses of Ontario.
- Knowledge of the healthcare delivery system, including hospital discharge planning and community care.
- Valid driver’s license and access to a reliable vehicle.
- Excellent case management skills and experience with interdisciplinary teams and interRAI assessments.
- Strong assessment, decision-making, and time management skills.
- Proficient in Microsoft Office and comfortable with new technologies.
