**Responsibilities:**
- Collaborate within an interdisciplinary team-based approach to care through a 16-week rehabilitation program at home
- Support a holistic and restorative approach to care via interventions, health coaching, and self-management support
- Utilize digital health platforms for care delivery, collaboration, documentation, and interventions
- Work primarily through in-person visits with potential for appropriate virtual visits
- Collaborate with PTA/OTA team members

**Qualifications:**
- Registered member in good standing with the College of Occupational Therapists of Ontario
- Commitment to person and family-centered care
- Strong communication skills and ability to collaborate within an interdisciplinary team
- Empathy, good judgment, and critical thinking skills
- Ability to travel within the Toronto community
